Welcome to the mental health integrated care pathways toolkit
An integrated care pathway (or ICP) is a
person-centred and evidence-based framework. It tells
multidisciplinary and multi-agency care providers, people using
services, and their carers what should be expected at any point
along the journey of care.
ICPs allow services to compare planned care
with what was actually delivered. This information can be used to
develop services and improve the patient journey.
This toolkit was developed primarily for
service providers working within, or in partnership with mental
health services, across all age ranges, in Scotland.
In this site you will find the national ICP
standards and their evidence base along with contacts, practical
help and resources. These will support you to develop and implement
mental health ICPs in your local area.
Bookmark us, and visit the toolkit regularly
as it will be updated with the latest ICP news and new resources as
they become available.
