Criterion
14a The care record shows:
• the diagnosis or diagnoses
• information on how the diagnosis or diagnoses was reached following evidencebased guidelines or established diagnostic criteria, where available
• confirmation that the diagnosis or diagnoses has been explained to the service user and informal carer, and
• post-diagnosis support is offered.
