Rationale
A particular diagnosis may suggest the
use of certain
treatments
and/or the likely course and outlook for
the child or young person. Information on how the diagnosis or
diagnoses was/were reached should be included in the care
record.
A diagnosis or diagnoses should be
recorded and explained to the child or young person and their
parents/carers, where appropriate. The explanation should include
access to information about the condition including the range of
treatment options available, as well as the support and resources
that may be available in the community.
Any diagnosis given should be reviewed regularly taking into
account the developmental growth of the child or young person. The
views of the child or young person and/or their parents/carers,
where appropriate, should also be taken into account.
